Vital Records is open by appointment and for walk-ins.

Monday by appointment only, and Tuesday-Friday 10am-noon open for walk-ins. Call for appointments. See below for additional order options.


We are accepting orders by MAIL sent to Vital Records at 151 W 7th Ave. #520. Requests submitted by mail should include a complete order form, a copy of photo identification, and payment in the form of a check or money order. Valid photo identification must include current address; if photo identification does not have current address, please include separate proof of address. Please DO NOT mail cash.

Orders can be dropped off at our DROP BOX outside the Vital Records office on the fifth floor of the Health & Human Services building (151 W. 7th Ave. #520). Requests submitted in our drop box should include a complete order form, a copy of photo identification, and payment in the form of a check or money order. Valid photo identification must include current address; if photo identification does not have current address, please include separate proof of address. Cash can be included in the envelope, it is a secure box.

Fees

Birth and Death Certificates Fees are $25.00 per certificate, as of January 1, 2018.

Birth Certificates for Babies under 5 Months and 28 Days

Lane County Vital Records is only able to print Certificates for babies born in Lane County within the first 5 months and 28 days from birth.

Birth Certificates may be requested by mail [NO in-person service currently, due to COVID-19 restrictions].

Birth Certificates for individuals born in Oregon more than 5 months & 28 days ago can be requested through Oregon State Vital Records.
