Springfield Schools Health Center provides health services to all Springfield School District students!









Services provided by the health center include:

Enyo Dzata MSN, PNP-BC

Dzata

Enyo earned her Bachelor of Science degree in Human Development and Psychological Services from Northwestern University in 2002. She completed her Health Sciences post‐baccalaureate from the University of Oregon in 2004 and earned her graduate degree in Nursing from Yale School of Nursing in 2007. Enyo is certified by the Pediatric Nursing Certification Board. Enyo joined our practice in 2016.

 

Enyo’s first job out of graduate school was in a pediatric specialty clinic in San Francisco for 3.5 years working in pediatric endocrinology treating children with Type 1 & 2 diabetes, growth hormone, thyroid, and pubertal conditions. She went on to take a position at The Children’s Hospital of Philadelphia in the Endocrine department for 5.5 years treating a rare condition in children that were admitted to this hospital from all over the country and internationally.

 

Enyo grew up in Oregon and attended high school in Eugene. After marrying and having a baby Enyo and her husband decided to be closer to family. She enjoys spending time with her family, outdoor activities and sporting events. Enyo is happy to be back in Oregon, working to keep kids healthy.


Belinda Burchell, M.Ed, LPC

Originally from Virginia, Belinda has settled in Oregon with her husband and their two children. Her driving force to relocate to Oregon was the overwhelming sense of community she experienced here. Belinda has worked with Lane County Behavioral Health and Community Health Centers of Lane County since 2013.

Springfield Schools Health Center is a School Based Health Center. All SBHC’s are required to administer the annual Student Satisfaction Survey to a sample of SBHC clients who are between the ages of 12 and 19. SBHC’s are provided the SPO-required Client Satisfaction Survey forms and instructions for their completion. SBHC’s must submit completed Student Satisfaction Survey data no later than June 30th of each year.

Parental Notification of SBHC Client Satisfaction Survey:

Students between the ages of 12 and 19 who visit the SBHC may be asked to complete an anonymous survey after their visit. The survey asks about the student’s satisfaction and experience at the SBHC, as well as some general questions about their physical and mental health status. Students can refuse to take the survey and this will not affect their ability to get care at the SBHC. A copy of the survey is available at the reception desk. Springfield Schools Health Center es un Centro de Salud Basado en la Escuela (School Based Health Center o SBHC). Todos los SBHC’s tienen el requerimiento de administrar anualmente una Encuesta de Satisfacción del Estudiante a una muestra de clientes de SBHC entre las edades de 12 y 19 años. Los SBHC’s son proporcionados los formularios de la encuesta e instrucciones para ser completados. Los SBHC’s deben entregar las Encuestas de Satisfacción del Estudiante a más tardar el 30 de junio de cada año. 

Notificación para padres sobre la encuesta de satisfacción del cliente de SBHC:

A los estudiantes de entre 12 y 19 años que visitan el Centro de Salud en la Escuela (School Based Health Center o SBHC) se les puede pedir que contesten una encuesta anónima después de su visita. La encuesta hace preguntas generales sobre su estado de salud física y mental. Los estudiantes pueden negarse a participar en la encuesta y esto no afectará su capacidad de recibir atención médica en el SBHC.
