Lane County Vital Records is only able to provide certificates for infants born in Lane County in the past 5 months & 28 days.
**Please note that the hospital will never order birth certificates from the county or state, it is up to the parents to do so**

US MAIL: Requests submitted by mail should include a complete order form, a copy of valid photo identification, and payment in the form of a check or money order. Valid photo identification must include current address; if photo identification does not have current address, please include separate proof of address.

DROP OFF: Orders can be dropped off in our DROP BOX outside the Vital Records office on the fifth floor of the Health & Human Services building (151 W. 7th Ave. Suite 100-1A). Requests submitted in our drop box should include a complete order form, a copy of valid photo identification, and payment in the form of a check or money order. Valid photo identification must include current address; if photo identification does not have current address, please include separate proof of address. Cash can be included in the envelope; it is a secure box.
EMAIL: Orders can be EMAILED to us at [email protected]. Requests submitted via e-mail should include a complete order form, a copy of valid photo identification. We will contact you to process payment by phone. Valid photo identification must include current address; if photo identification does not have current address, please include separate proof of address.
IN PERSON: Orders may be placed in person at our Vital Records office on the fifth floor of the Health & Human Services building (151 W. 7th Ave. Suite 100-1A). Our walk-in hours are Monday – Friday from 10 AM to noon and from 1 PM to 4 PM.
