Scattered site PSH providers will need to submit this referral form to Lane County with the appropriate attachments, in order to be considered for a Housing Choice Voucher via the PSH “Move On” process. Attachments include a copy of the Move On Matrix, a HUD 29006 Release of Information for and a signed ROI from your agency that lists Lane County Human Services Division, Homes for Good to allow for the communication of updates.
