The 2024 Community Health Assessment is currently underway and is being led by the Community Health Assessment Design Team. More information will be placed here as we make progress. Live Healthy Lane Vision:
The first job of the Assessment Design Team was to update Live Healthy Lane’s vision for a healthy and equitable community. After community conversations hosted around Lane County in June 2024, the Design Team developed this vision statement which will guide the 2024 Community Health Assessment:

We envision Lane County as a welcoming community. A place where systems - social, economic, and environmental, etc. - are developed by everyone to work for everyone. A community where all people have what they need to thrive. This includes: a sense of belonging, a feeling of shared purpose, and hope for our collective future.

Community Status Assessment – Help us review the data! These data are a draft and should not be used for data-driven decision-making at this time. The 2018-2019 Lane County Community Health Assessment (CHA) is more than charts, graphs, statistics and numbers. It is a comprehensive assessment that engages the community to help us understand not only what health conditions we should be concerned about, but what are the conditions of our community that both support and hinder good health for everyone.

There are multiple components to this assessment, each of which answers a different set of questions:

• The Local Public Health System Assessment (LPHSA) answers questions about the capacity and strength of Lane County’s public health system.
• The Forces of Change Assessment (FOCA) answers questions about what forces beyond our local control do have or could have impacts on the health of our community.
• The Care Integration Assessment (TCIA) answers questions about how well our health systems are integrating care for physical health, behavioral health, and social needs. 
• The Community Themes and Strengths Assessment (CTSA) answers questions about what community members think is important for health and what assets Lane County has to support good health. 
• The Community Health Status Assessment (CHSA) answers questions about what health conditions are significant in Lane County and the state of social conditions (e.g. education, employment, housing, etc.) that influence health. (A summary of the full report is also available.)
• The Lane County Health Equity Report (LCHER) answers questions about inequities in health and social conditions in Lane County that are the result of unjust systems and histories of oppression.

The information from the Community Health Assessment is reviewed by Live Healthy Lane and community partners to identify common themes and root causes. That information is then shared with the community to identify priorities and develop strategies that become Lane County's Community Health Improvement Plan (CHP). 

Lane County’s 2021-2025 CHP has one goal: 

Create the community conditions necessary to promote behavioral health and physical wellness across the lifespan for all people in Lane County.

Based on the findings in the CHNA, three priorities and four strategies were identified to improve the community’s health:

• Ensure everyone has the income needed to meet basic needs
  • Support economic development that creates livable incomes and keeps basic living costs affordable
• Create conditions that support good mental health and physical well-being
  • Ensure systems of care address the needs of the whole person across their life-span
  • Implement policies that support healthy choices and mental well-being
• Address the injustices that create inequities
  • Ensure equity in the future by transforming the systems and structures of our racist past

Live Healthy Lane, in consultation with community partners, has developed a shared set of indicators that the community can use to track progress on the CHP priorities. The indicators are available in English and Spanish.

The challenges Lane County faces in creating optimal conditions for good health are more than any one organization or agency can take on alone. That’s why we use a collective impact model in our approach to addressing the priorities in the CHP.
